Hello to all,
I am new to the boards and first want to say hi to everyone here. A little history before i post my results. I have been in 3 MVA all with whiplash. I have had 3 Cervical spine surgeries,I have a titanium plate with hardware at c4-5-6-7, 1 staph infection and 1 Lumbar surgery. After my staph infection in July 2003, I was left with a terrible paraspinal pain that was uncontrollable. I have been treated with pain management, Pt, and chiro for trigger point therapy only. My left shoulder now at this point is beyond the point of being in pain. I am on duragesic patches, oxycodone, topamax and still am having pain 24/7 . I Also have an enlarged lymph node in my neck that is being "watched" by my surgeon and I will be posting my results of my c-spine MRI on the spinal boards. So the MRI findings are as follows: ( sorry this is so long)
small punctate ganglia are noted at the posterior aspect of the proximal humerus. There is infraspinatus tendinosis w/ a low grade small articular sided partial distal tear. There is supraspinatus tendinosis w/thickening & mild high signal intensity, most pronounced far anteriorly; small low grade partial intrasubstanve tendon tears are noted as well as fraying at the bursal surface. The subcapularis tendon is intact. The long head of the biceps tendon is maintained in position and unremarkable. There is no denervation effect. Thickening and slight fluid distenion of the subacromial/subdeltoid bursa is noted. There is a bulky midline glenohumeral ligament & a deficient anterosuperior labrum consistent with a Buford complex. A sublabral sulcus is noted superiorly. No labral tear. A small amount of fluid is noted in the joint and subscapularis recess.
CONCLUSION: MODERATE SUPRASPINATUS AND MILD INFRASPINATUS TENDINOSIS, SMALL INTRASUBSTANCE LOW GRADE PARTIAL INFRASPINATUS AND SUPRASPINATUS TENDON TEARS, AND FRAYING ALONG THE BURSAL SURFACES OF THE SUPRASPINATUS WITH ADJACENT MILD SUBACROMIAL/SUBDELTOID BURSITIS
I have had this pain for over 2 years now. I use ice packs daily and chiro treatment 2-3 times a week and still have no relief. I have read several articles on this topic and majority of them state that with pt etc, the pain can be relieved without surgery. As you can see, that is not working. What is my option and could you tell me what these finding mean in English??? My appointment isn't till the end on November and I am a nervous wreck. Thank you so much.

This is the sort of thing that really needs to be evaluated by an orthopedic surgeon, in person. I'm happy that you have an appointment; let's wait for that, and let us know how it goes!
